Look for the guides with YELLOW umbrella in front of Museo 900, Duomo Square.
Underground Stop: Duomo (yellow underground - linea 3).

Things to note

You have to show up at least 5 minutes from the starting time or your booking will be canceled.
It is not allowed to book for more than 4 people. All the bookings to circumvent the limit number will be considered null (eg. 2 bookings for one group)
